Participant’s Agreement: I/my son named above agree that I/my son named above will be cooperative and comply with the Coaches’, the MAS Sports League Administration and volunteers” instructions at all times for my own safety. Non-compliance will result in having my parent being called if I am a minor and my dismissal from playing. This will also ban me from my participation in any future events organized by MAS Sports League.

By submitting this form, I hereby consent and approve that my son named above/I (adult) can participate in all the athletic practices and activities as organized by the Coach. I understand that MAS Sports League Administration, the Coaches and Volunteers are not responsible for any damage or injury that occurs during my son’s participation in any of the practices and activities.
